iBridge Expert Solutions is currently recruiting an Aboriginal Health Worker / Aboriginal Health Practitioner to join a supportive, multidisciplinary primary healthcare team.
This is an exciting opportunity to make a meaningful impact by delivering culturally safe, person-centred healthcare and improving health outcomes for Aboriginal and Torres Strait Islander communities.
What You’ll Be Doing
◆ Support culturally safe primary healthcare delivery.
◆ Assist with health assessments, including 715 Health Checks.
◆ Support chronic disease management, care planning, and follow-up.
◆ Work alongside GPs, Nurses, and Allied Health professionals in providing quality patient care.
◆ Build trusted relationships with Aboriginal and Torres Strait Islander clients and support them in accessing healthcare services.
◆ Deliver health promotion and preventative health initiatives.
◆ Maintain accurate clinical documentation and contribute to quality improvement activities.
About You
Essential
🔹 Identify as an Aboriginal and/or Torres Strait Islander person (Identified Position).
🔹 Certificate III in Aboriginal and Torres Strait Islander Primary Health Care (or nearing completion).
🔹 Certificate IV in Aboriginal and Torres Strait Islander Primary Health Care (Health Practitioner), or willingness to work towards obtaining it.
🔹 Strong understanding of health issues affecting Aboriginal and Torres Strait Islander communities.
🔹 Excellent communication and interpersonal skills.
🔹 Ability to work collaboratively within a multidisciplinary team.
🔹 Current Open C Class Driver Licence.
🔹 Current CPR certification.
🔹 Ability to obtain a Blue Card and National Police Check.
Desirable
◆ Experience working in Aboriginal Community Controlled Health Organisations (ACCHOs) or Aboriginal Medical Services.
◆ Experience with chronic disease management and health assessments.
Knowledge of Best Practice or similar clinical software.
◆ Background in health, aged care, disability, or community services.
